Required Documentation Per Course
Prerequisites for Enrollment:
- Must be 18 years of age, before beginning the course;
- Must have a current active Kansas CNA certificate, without prohibitions;
- Must have computer and internet access to complete the online portion of the class.
Required for enrollment:
- Two Personal Documents
- Current state-issued photo ID and Social Security card
- Report of Tuberculosis (TB) Skin Test OR TB Chest X-Ray Report)
- Must be completed within the past 6 months
- Information about testing can be found on Riley County Health Department's website.
- Flu Immunization for the Current Season
- Information about the flu vaccine can be found on the Riley County Health Department's website.
- Evidence of COVID-19 Vaccination
- Required by the clinical facilities.
- Kansas CNA number
- Information can be found at www.ksnurseaidregistry.org
- CASAS Or TABE Reading Test OR Proof of College-Level Writing Course
- Schedule Exam $5.00 fee will need to be paid upon scheduling the exam
- If you have taken a college-level writing course (English Composition, Expository Writing, etc.) and passed with a grade “C” or higher you will need to submit your transcript or grade report.

Pre-Requisites for enrollment:
- Prospective students must hold the Kansas Certified Nurse Aide (CNA) credential, but are not active (certification has lapsed) and want to reactivate their CNA status.

- Applicants must be at least 17 years of age by the start of the course.

MMR Documentation
- Two (2) doses or proof of immunity by titer
Chickenpox (Varicella)
- Two (2) doses or proof of immunity by titer
- WILL NOT ACCEPT HISTORY OF CHICKENPOX - Chickenpox is an immunization that is often incomplete. We can only accept proof of vaccination or a titer. We cannot take anyone’s testimony that you had chickenpox, not even a physician. Make sure that you have either vaccines or a titer. Paperwork for a low titer will not be accepted.
Tetanus (TDAP)
- One (1) dose taken less than 10 years ago - Please ask your healthcare provider to give you the TDAP vaccine.
Hepatitis B (Hep B)
- Three (3) doses or proof of immunity by titer. If your titer is low, you should get the vaccine. Paperwork for a low titer will not be accepted.
- Report of Tuberculosis (TB) Skin Test OR TB Chest X-Ray Report)
- Must be completed within the past 6 months
