Job Description:



  1. Record all entries on flow sheets, notes, point of care, and other resident medical record documents in an informative and descriptive manner.
  2. Report all accidents and incidents you observe to the charge nurse on the shift that they occur.
  3. Use only authorized abbreviations established by this facility when recording resident medical record information.
  4. Report all changes in residents’ conditions to the Charge/Staff Nurse as soon as practical.


  1. Ensure that the resident’s room is ready to receive the resident.
  2. Greet and escort resident to his/her room.
  3. Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate.
  4. Obtain resident’s weight, height, and vital signs.
  5. Make resident comfortable (i.e., get water, put to bed).
  6. Inventory and mark the resident’s personal possessions.
  7. Store resident’s clothing.
  8. Assist residents with packing their personal possessions when they are being transferred to a new room or when being discharged.
  9. Transport residents to their new room and assist with storing their personal possessions.


  1. Follow work assignments and/or work schedules in completing and performing assigned tasks, and be willing to help others with their assignments.
  2. Perform all assigned tasks in accordance with Gove County Medical Center’s established policies and procedures and as instructed by supervisors.
  3. Notify the facility when you will be late or absent from work in accordance with established policies and procedures.
  4. Report all complaints and grievances made by residents to the charge nurse.
  5. Cooperate with inter-departmental personnel, as well as other facility personnel, to ensure that nursing services can be adequately maintained to meet the needs of the residents.
  6. Create and maintain an atmosphere of warmth, personal interest, positive emphasis, and a calm environment throughout the LTCF and shift.
  7. Meet with your shift’s nursing personnel on a regularly scheduled basis to assist in identifying and correcting problem areas and/or the improvement of services.


  1. Participate in and receive the nursing report upon reporting for duty.
  2. Assist residents with daily dental and mouth care.
  3. Give or assist residents with bath functions (bed bath, tub, or shower).
  4. Assist residents with dressing/undressing as necessary.
  5. Assist residents with grooming (hair care, foot care, hand and face washing).
  6. Provide oral hygiene and denture care.
  7. Assist residents with nail care (cleaning, trimming, and filing).
  8. Shave male residents.
  9. Change residents’ bed linens, make beds, and place extra covers on beds as needed.
  10. Maintain intake and output records as instructed.
  11. Assist resident with bowel and bladder functions (i.e., offer urinal, bedside commode).
  12. Provide perinatal care.
  13. Keep incontinent residents clean and dry.
  14. Check and report bowel movements and character of stools as instructed. Document in POC.
  15. Collect specimens as instructed.
  16. Change oxygen tubing, cannulas, humidifiers, and filters per protocol.
  17. Assist in transporting residents to/from meals, activities, and social programs.
  18. Answer residents’ call lights promptly.
  19. Measure and record residents’ temperatures, pulse, and respirations as instructed. Document in POC.
  20. Weigh residents at bath time and document results in POC.
  21. Check residents routinely to ensure that their personal care needs are being met.
  22. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, whirlpool, wheelchairs, and lifts.
  23. Assist residents to walk with or without self-help devices as instructed.
  24. Assist residents with use of walkers, wheelchairs, canes, and braces.
  25. Perform restorative and rehabilitative procedures as instructed.
  26. Provide eye and ear care (i.e., cleaning eyeglasses/hearing aids).
  27. Provide post-mortem care as instructed.
  28. Respond to door alarms.
  29. Follow Infection Control policies.
  30. Follow Care Plans.


  1. Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers.
  2. Provide daily perinea care.
  3. Assist with sponge baths.
  4. Provide range of motion exercises as instructed.
  5. Turn bedfast residents at least every two (2) hours.
  6. Observe disoriented and comatose residents. Record and report data as instructed.
  7. Watch for and report any change in room temperature, ventilation, or lighting.


  1. Prepare residents for meals (i.e., wash hands, comb hair, take to/from dining room).
  2. Serve food trays. Assist with feeding as indicated.
  3. Assist residents with sight problems by identifying food arrangements (i.e., what foods are on tray, where it is located, if it is hot/cold).
  4. Record residents’ food/fluid intake. Report changes in residents’ eating habits.
  5. Keep residents’ water pitchers clean and filled with fresh water and placed within easy reach of residents.
  6. Serve between meal and bedtime snacks. Document in POC.
  7. Perform after meal care (i.e., remove trays, clean residents hands, face, clothing, brush teeth, clean dentures).
  8. Check rooms for food articles (i.e., food in proper container, unauthorized food items).


  1. Attend and participate in scheduled training and educational classes.
  2. Attend and participate in scheduled orientation programs and activities.
  3. Attend LTC staff meetings.


  1. Use restraints when necessary and in accordance with Gove County Medical Center’s policies and procedures.
  2. Check restrained residents every thirty (30) minutes.
  3. Release restraints every two (2) hours.
  4. Notify the charge/staff nurse of any residents leaving/missing from the facility.
  5. Keep floors dry. Clean spills immediately.
  6. Keep excess supplies and equipment off the floor. Store in designated areas.
  7. Keep residents’ personal possessions off the floor and properly stored.
  8. Keep the nurse call system within easy reach of residents.
  9. Report all hazardous conditions and equipment to the charge/staff nurse immediately.
  10. Follow established safety precautions in the performance of all duties.
  11. Report all safety violations.
  12. Before leaving work area for breaks, or at the end of the workday, store all tools, equipment, and supplies.
  13. Wash hands before and after performing any service for residents.
  14. Wash wheelchairs, walkers, and lifts as instructed.
  15. Clean, disinfect, and return all resident care equipment to its designated storage area after each use.
  16. Perform routine housekeeping duties (i.e., clean residents’ drawers, closets, and night stands).
  17. Wash hands before entering and leaving an isolation room/area.
  18. Report any communicable or infectious disease that you contact to the Director of Nursing Service.
  19. Follow established isolation precautions and procedures. Use gowns, gloves, and surgical
  20. masks as indicated.
  21. Use proper lifting techniques at all times.
  22. The use of a gait belt, Sara Lift, or Dextra Lift is mandatory when lifting, moving, or transferring any resident who has been identified as needing assistive lifting devices.
  23. Employees under 18 years of age may assist with the mechanical lifting devices such as the Sara Lift and the Dextra Lift.


  1. Use only the equipment you have been trained to use.
  2. Operate all equipment in a safe manner.
  3. Use only the equipment and supplies necessary to do the job.
  4. Report defective equipment to the charge/staff nurse immediately.
  5. Inform the charge/staff nurse of your equipment needs.
  6. Keep equipment, supplies, and resident furniture clean.
  7. Keep environment clean.


  1. Review care plans daily to ensure that you are providing appropriate nursing care.
  2. Inform the charge/staff nurse of any changes that need to be made on the care plan.
  3. Ensure that your notations reflect that the care plan is being followed.


  1. Maintain the confidentiality of all resident care information.
  2. Ensure that you treat all residents fairly, with kindness, dignity, and respect.
  3. Ensure that all nursing care is provided in privacy.
  4. Knock before entering residents’ rooms.
  5. Report all grievances and complaints made by the resident to the charge/staff nurse and DON.

  1. While performing the duties of this job, the employee is regularly required to walk, sit, stand, bend, lift, push, pull, kneel, stoop, use hands to grip and handle objects, tools, or controls. Must be able to reach with hands and arms.
  2. Must be able to speak clearly and hear with or without the use of corrective devices.
