Job Description

  • Review immunization status during assessments and coordinate needed immunization administration. If unable to complete, refers to local health departments for immunization updates. 
  • Note any physician’s orders and ensure compliance. 
  • Track and monitor clients with chronic medical conditions to ensure continuity of care. 
  • Administer medications safely and accurately, following all regulatory and facility guidelines. 
  • Document all nursing assessments, interventions, complete nursing documentation, immunization administration, medication administration records (MARs) timely and accurately in the electronic health record (EHR) in accordance with facility, state, and accreditation standards. 
  • Obtain and document informed consent for new medications in accordance with regulatory and organizational requirements. 
  • Provide medication education to clients, including purpose, benefits, and potential side effects. 
  • Perform routine and as‑needed nursing procedures such as vital signs, blood glucose checks, wound care, and first aid. 
  • Obtain lab specimens as scheduled and perform phlebotomy to obtain the specimens as needed. 
  • Refer medical or psychiatric concerns to appropriate professionals. 
  • Participate in crisis intervention, including verbal de‑escalation and coordination with clinical staff during behavioral emergencies. Ability to administer intramuscular injections as needed during crisis.  
  • Provide nursing monitoring during and after crisis events, including seclusion or restraint, ensuring compliance with trauma‑informed and regulatory standards. 
  • Monitor all seclusion and restraint incidents for safety, adherence to policy, and proper documentation. 
  • Educate clients on health topics such as hygiene, nutrition, medication adherence, and chronic condition management. 
  • Communicate effectively with caregivers, case managers, and external providers to support coordinated care. 
  • Capable of being on call 
  • Report unusual incidents through appropriate Risk Management, clinical, and safety channels. 
  • Ensure clients’ rights are protected and always upheld. 
  • Is knowledgeable of and follows all safety procedures.  
  • Implements Saint Francis Ministries policies and follow directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP’s), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state, and local rules, regulations and contractual terms. 

Qualifications:

  • Requires high school diploma or equivalent. 
  • Requires Licensed Practical Nurse (LPN).
