Dependent students selected for the V5 federal verification process must complete and submit this form. Please note that we strongly encourage students and their parents to use the IRS Data Retrieval Tool. If it was not used when the initial application was made, parents and student may go back into the FAFSA to make corrections and use the Data Retrieval Tool. Otherwise the parents and student will need to contact the IRS using the instructions on page 2 of the Verification Form to request the 2016 Tax Return Transcript for the 2018-2019 school year.
